Station Self Survey
To initiate the process we will forward to you, upon the completed form below, a budgetary quotation for a Plymovent Emergency Vehicle Exhaust System to use in your 2023 AFG Grant Application.
Please complete the survey as accurately and completely as possible.
If your department has more than one station, please complete separate surveys for each station.

Note as indicated in the Station Floor Plan:
- Distance from tailpipe discharge to garage door
- Side of the discharge of the exhaust system
- Width and height of the garage bay doors
- Voltage of electrical supply to apparatus bay
Please also note whether the exhausts are under carriage or vertical stack and if the stack is single or dual.
